IELTS for doctors: the GMC 7.5 rule, measured

The GMC requires IELTS Academic with an overall 7.5 and at least 7.0 in each of the four skills, from the same test. Leo enumerated every possible score report: only 117 of the 2,059 reports with an overall 7.5 pass, and four straight 7.0s fail because they round to 7.0 overall.

Doctors registering with the General Medical Council (GMC) need IELTS Academic with an overall band of at least 7.5 and at least 7.0 in each of the four skills, all from the same test. The second condition is the one that decides most results: Leo’s count of every possible score report shows that only 5.7 % of reports with an overall 7.5 also clear 7.0 in every skill.

The GMC rules below were read on gmc-uk.org on 11 October 2026. The GMC writes that the evidence it accepts “is subject to change”, so confirm the current page before you book a test.

What IELTS score does the GMC require?

The GMC requirement for IELTS has five conditions, and a certificate must meet all of them: the Academic test, at least 7.0 in each testing area, an overall score of at least 7.5, all achieved in the same test, and all achieved in your most recent sitting. In the words of the GMC’s IELTS guidance for doctors:

Be careful with summaries. The IELTS directory entry for the GMC shows “7.0” as its headline minimum band score, while its own text underneath says 7.5 overall. The overall is the number that is easy to misread. A 7.0 in each skill sits at C1 on the official alignment, the level described on our C1 English page.

Why does an overall 7.5 usually fail the GMC rule?

An overall IELTS 7.5 usually fails the GMC rule because the overall is an average, and an average of 7.5 leaves plenty of room for one skill at 6.5 or below. Leo measured how much room by enumerating all 130,321 possible IELTS score reports, applying the official rounding rule to each, and testing both GMC conditions.

The space is finite, so the count is exact. It is the same enumeration we used in what is a good IELTS score, run with the GMC rule:

According to Leo’s analysis, 2,440 of the 3,060 reports with an overall of 7.5 or more fail the 7.0 floor, which is 79.7 %. Even an overall 8.5 can fail: 6.5, 9.0, 9.0, 9.0 averages 8.375, rounds to 8.5, and is refused.

These are possible reports, each counted once, not real candidates. Real profiles are less lopsided, so we reran the count with every skill between 4.0 and 9.0. The pass rate at an overall 7.5 moves only from 5.7 % to 6.1 %.

Why are four scores of 7.0 not enough?

Four IELTS scores of exactly 7.0 are not enough for the GMC because they average 7.0, and the GMC also requires an overall of 7.5. Leo’s enumeration found exactly five reports that clear 7.0 in every skill and still fail on the overall: 7.0, 7.0, 7.0, 7.0 and the four profiles with a single 7.5.

The rounding rule decides the rest. IELTS rounds an average ending in .25 up to the next half band, so you need two half bands above 7.0 in total:

In practice the overall condition almost never binds on its own: of the 625 reports with 7.0 everywhere, 620 pass. The floor does the filtering. Aim for 7.0 in your weakest skill and 7.5 in two others, and the overall takes care of itself.

How is the GMC rule different from the NMC rule?

The GMC rule differs from the NMC rule on three points: the GMC sets an overall minimum of 7.5, it asks for 7.0 in writing rather than 6.5, and it does not let you combine two sittings. For couples or colleagues preparing together, the same score report can register a nurse and be refused for a doctor.

We counted it. Of the 750 possible reports that meet the NMC floors, 130 are refused by the GMC (17.3 %): 115 because writing is 6.5, 10 for writing and the overall together, and 5 because the overall is 7.0. The NMC side is explained in IELTS for nurses.

Can you combine tests or retake one skill?

You cannot combine two IELTS tests for the GMC, but you can use IELTS One Skill Retake once to repair a single section. The GMC accepts a retake in the section where you missed the minimum if the full test was taken on computer at a centre offering it, and you sit the retake within 60 days.

One Skill Retake is not available after an IELTS Online or paper test. If you plan to rely on it, book the computer-delivered test at a participating centre from the start.

How long is an IELTS certificate valid for the GMC?

An IELTS certificate is valid for two years for the GMC, and the date that counts depends on the step. For PLAB, it must be valid on the day you sit PLAB part 1. For registration, it must be valid on the day the GMC approves your application.

There is one extension: if the certificate is under three years old and you passed PLAB 2 less than three months before your registration application was received, the GMC may accept it. What about OET for doctors?

OET is the GMC’s other accepted test, and the rule mirrors IELTS: the Medicine version, at least grade B in each of the four sub-tests, the same test, your most recent sitting. The GMC’s OET guidance accepts OET on paper, on computer and OET@Home.

OET uses clinical scenarios, such as a referral letter that begins “I am writing to refer Mrs Patel, a 64-year-old retired teacher, for further investigation”. Whether that suits you better than academic topics is a personal call; the per-skill floor applies in both.

How should doctors plan their IELTS target?

Doctors should plan an IELTS target skill by skill: 7.0 is the floor in each, and the overall takes care of itself only if two skills reach 7.5. The skill at risk is whichever sits lowest on your practice tests, and that is where the extra hours belong.

Book IELTS Academic on computer at a centre that offers One Skill Retake, so a single weak section can be repaired within 60 days.

Time the test against approval, not application. The certificate has to be valid when the GMC approves your registration.

What IELTS score do doctors need for the GMC?
The General Medical Council requires the Academic version of IELTS with an overall band of at least 7.5 and a score of at least 7.0 in each of the four testing areas: listening, reading, writing and speaking. Both conditions apply at once, and the scores must come from the same test and from your most recent sitting. IELTS Online and IELTS One Skill Retake are accepted, the IELTS Indicator test is not. The same minimum applies whether you are booking PLAB part 1 or applying for registration with a licence to practise, so one certificate can serve both steps if it is still valid.
Is an overall IELTS 7.5 enough for GMC registration?
Usually not on its own, because the GMC also requires 7.0 in every skill. Leo counted every possible IELTS score report and applied the official rounding rule: of the 2,059 reports that produce an overall 7.5, only 117 have 7.0 or more in all four skills, which is 5.7 %. Restricting to realistic reports with every skill at 4.0 or above barely changes it, at 6.1 %. A typical failing profile is 8.5, 8.0, 6.5 and 7.5, which averages above 7.5 and still misses the writing floor. Check the lowest line of your report before the overall.
Can I combine two IELTS tests for the GMC?
No. The GMC guidance says you must achieve the required scores in the same test and in your most recent sitting, so a strong listening score from March cannot be paired with a strong writing score from June. The one exception is IELTS One Skill Retake. If you miss the minimum in one section of a full test taken on computer at a centre that offers it, the GMC will accept a One Skill Retake in that section, once, if you sit it within 60 days of the full test. This is stricter than the NMC, which accepts results combined from two sittings.
Do I need 7.5 in writing for the GMC?
No. The writing minimum is 7.0, the same as listening, reading and speaking. The 7.5 applies only to the overall band, which is the average of the four scores rounded to the nearest half band. That creates a trap: four scores of exactly 7.0 average 7.0 and fail the overall requirement, even though every single skill meets its floor. You need at least two half bands above 7.0 somewhere, for example 7.5 in listening and 7.5 in reading, to lift the average to 7.25, which IELTS rounds up to 7.5. Writing at 7.0 is fine if the others carry the average.
How long is an IELTS certificate valid for the GMC?
Two years. If you are booking PLAB, the certificate must still be valid on the day you sit PLAB part 1. If you are applying for registration, it must be valid on the day the GMC approves your application, not the day you apply, so a slow application can outlive a certificate. The GMC may extend validity for registration if the certificate is less than three years old and you passed PLAB 2 less than three months before your application was received. That extension does not apply to the sponsorship route. Check the dates before you book a retake or pay a fee.
Does the GMC accept OET instead of IELTS?
Yes. The GMC accepts the Occupational English Test if you took the Medicine version and achieved at least grade B in each of the four testing areas: speaking, listening, reading and writing. As with IELTS, the grades must come from the same test and your most recent sitting, and the certificate is valid for two years. OET on paper, OET on computer and OET@Home are all listed as accepted. OET uses medical material, which some doctors find more familiar, but the per-skill floor works the same way: one weak sub-test is enough to fail, whatever the other three say.
